OUR PROGRAMS For more information on any of our programs, call Street Health at (416) 921-8668. Nursing Program - Staff nurses work with volunteer RNs and Nurse Practitioners to operate scheduled clinics in drop-in centres and shelters. Nursing outreach is designed to locate people living on the street who generally avoid mainstream services. Mentoring of nursing students and other health care related disciplines, and conducting public education, are important components of our program. Community Mental Health Program - Street Health's community mental health program provides support for people who are homeless or under-housed and experiencing mental health issues. The staff assist people to access basic needs such as shelter and housing, income, health care, and personal identification. Partnerships with neighbouring service providers continue to be an important part of the Mental Health Program. For example, we provide case support at the Fred Victor Centre and the Good Neighbours Club. NEW Crisis Outreach Services for Seniors (COSS) - The COSS project is aimed at providing mobile crisis intervention and outreach to seniors (65+) in south east Toronto who have mental health and addictions issues and who are not connected to other social services. The program provides mobile crisis intervention and outreach, 7 days a week in the community, in homes, drop-ins, and in community centres. COSS operates in partnership with, COPA, South Riverdale Community Centre, the Good Neighbours' Club, and Wood Green Community Services. NEW The Safer Stroll Project - The Safer Stroll peer outreach and educational program is aimed at breaking down barriers women working in street based sex work face when attempting to access health and social services. The program provides educational workshops on topics such as self defence and healthy lifestyle choices. The project furthers the development of stronger peer-led networks for sex workers.
Harm Reduction Program - The street outreach team provides clients with education, referrals and peer support related to the harms associated with substance use. The program includes needle distribution, safer crack kit distribution, street outreach service, and support for sex workers.
NEW The Drug Information Project - The Drug Information Project is a partnership between Street Health and Regent Park Community Health Centre aimed at providing coordinated services to individuals who use substances and who may not otherwise access support and/or medical care. The Project provides a weekly drop-in group with easy access to health care, social workers, housing help, peer education and support, healthy food, community information and referrals, as well as peer training opportunities. Both agencies work collaboratively to promote these services to our community resulting in a very lively and diverse drop-in group. The project is a harm reduction initiative that achieves positive health outcomes among individuals who often struggle with very complex health and social concerns.
I.D. Replacement - Access to Health Cards for the Homeless - Street Health's Access to Health Cards program was established to assist homeless people with the sometimes onerous task of applying for health cards and the other personal identification they need to access health and other essential social services. ID Safe - Homeless and under-housed individuals can easily lose their identification due to unstable living conditions. The ID Safe program at Street Health offers security for homeless people by storing their important identification documents which in turn assists them to maintain access to services which require proof of identification. (Click here for more information) Mail Service - Homeless people by definition lack the postal address required to qualify for certain services. Street Health will provide the necessary mailing address and handle a person's mail. Community Based Research - Research at Street Health continues to be a valuable source of information and a tool for advocacy for Street Health and for other community agencies and coalitions working to address issues of homelessness. Clothing and Sleeping Bag Distribution - Clean, donated clothing and sleeping bags are distributed to those in need.
