Street Health is a community-based agency providing innovative physical and mental health programs and advocacy to
homeless and insecurely housed individuals in downtown Toronto for over 30 years. Street Health, a not-for-profit
organization located within the downtown-east neighborhood of Toronto works to improve the health and wellbeing of
people who are homeless and under-housed by addressing the social determinants of health. Street Health provides
services in the areas of nursing, mental health, harm reduction, overdose prevention, Safer Supply, outreach, identification
replacement programs, and through advocacy, research, and education.


This position will support the Safer Opioid Supply (SOS) Program, which is an initiative that takes place in collaboration with
other downtown-east Toronto community-based agencies and provides essential services to people who use substances
by providing prescribed alternatives and managing the comprehensive healthcare of clients.

 

Responsibilities of the position include, but may not be limited to:

 

• Deliver primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of
Nurses of Ontario (CNO) and Bill 179, including comprehensive health assessments for those who use substances;
• Deliver primary care services, traditional OAT, and safer supply to people who use drugs, in a client-centered, harm
reduction-focused manner;
• Contribute to the development of the safer supply program at Street Health;
• Provision of primary healthcare grounded by the five principles of Primary Healthcare as outlined by the World
Health Organization;
• Monitor client health status and investigation results;
• Identify barriers to health and facilitate access to resources;
• Deliver client education through individual and group health education sessions;
• Collaborate and consult with multidisciplinary staff within Street Health and partnering organizations to facilitate
comprehensive primary care provision;
• Act as a preceptor to NP and Registered Nursing (RN) students and mentor new staff members and/or students;
• Collaborate with other colleagues in this program to provide coverage for vacation/sick time and follow-up client
care;
• Provide care at various drop-in locations and participate in outreach activities;
• Engage in evidenced based practice and ongoing learning;
• Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies
to meet client needs; collaborate with colleagues to identify population trends, develop research questions, and/or
conduct research studies;
• Advocate for individual client needs to ensure access to effective care, follow-up, and resources from both an
individual and systemic level;
• Participate in the evaluation of NP practice and the ongoing development and improvement of the NP role within
Street Health; contribute to the overall policy and program directions of Street Health;
• Facilitate knowledge translation by educating colleagues on topics related to client care;
• Initiate and/or participate in systemic advocacy activities directed at achieving health equity by improving health
service delivery, eliminating stigma and developing healthy public policy; and
• All other duties as may be assigned within the scope of this position.

 

Qualifications & Requirements:

Master’s Degree in Nursing.
• Current registration as a Nurse Practitioner, in good standing with the College of Nurses of Ontario.
• Minimum five years of nursing experience including demonstrated knowledge and commitment to the principles of harm
reduction.
• Experience working within a community-based, not-for-profit organization focused on nursing outreach, harm reduction, and mental health supports.
• Completion of the Opioid Dependence Treatment Course via CAMH or the Opioid Addiction Treatment Support Program
via BCCSU within one month of the start date.
• Demonstrated knowledge of mental health diagnosis, substance use treatment, harm reduction modalities, and social
determinants of health.
• Experience advocating for clients, individually and at a systemic level.
• Strong clinical assessment and documentation skills.
• Strong oral and written communication skills; strong computer skills and experience using EMR software.
• Experience working with marginalized groups and a strong understanding of systems and issues affecting individuals who
experience mental health diagnoses and homelessness.
• Experience working within a team/organization harm reduction approach.
• Certification in Advanced Nursing Footcare, or interest and willingness to attend training.
• Ability to carry out duties independently while maintaining accountability to the agency.
• Valid CPR certification or ability to obtain certification within the first 3 months of employment.
• Successful police services Vulnerable Sector Screening Check.
• Availability to participate and provide on-call rotation support to clients, including evenings and weekends.

 

Position Type: Full-time, Contract, Bargaining Unit
Salary: $62.56 per hour as per the Collective Agreement
Benefits: Participation in the organization’s group health & dental coverage plan (100% employer
paid), and the organization’s Pension Plan, MSPP.
Work Hours: 37.5 hours per week, Monday to Friday
9:00 am to 5:00 pm (some adjustments may be needed to meet the needs of the
organization).
